Food-contact surface soiled with food debris, mold-like substance or slime. Can opener blade soiled. Located at the end of the prep table in the kitchen area. The can opener was removed to be washed, rinsed and sanitized.

Handwash sink not accessible for employee use at all times. Hand washing sink is blocked by the door being propped open. Adjustments was made to how wide the door was open.
