Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Operator moved breaker to correct side of splitter.
Employee with no hair restraint while engaging in food preparation. Employee and manager have no restraint.
Food stored on floor. Box of fries and bag of onion rings on freezer floor.
