Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Dish washer observed cleaning dish machine equipment with wet rag and immediately began putting away clean equipment. No hand washing in between. Educated on site.
