Operator rearranged for proper storage.
Stop Sale issued due to food not being in a wholesome, sound condition. Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. In the dinner and lunch menu, raw tuna and raw salmon is served raw and it is not correctly identified on menu. Operator began marking raw menu items indicating the consumer advisory.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On the cook line, corned beef (41F - Cold Holding). Per operator the corned beef has been out of the cooler for one hour. Operator moved the corned beef back into the cooler and iced it.
Ice scoop handle in contact with ice. There is a ice scoop with the handle in contact with the ice at the bar. Operator moved ice scoop to proper storage.

Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. In the double door reach in cooler across from the triple sink there is lasagna and vodka sauce with no date marking that were prepared more than 24 hours ago per the operator.
Bowl or other container with no handle used to dispense food. There are plastic to go containers inside bins of rice, semolina, flour, tempura batter and potato starch. Operator removed the to go containers.
Employee eating while preparing food. Employees are eating while preparing food in the prep area by the dish washing area. Inspector educated the employees and the operator.
