Employee used the bathroom and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands upon returning to employee area. Observed employee come out the bathroom, placing on gloves and grab a roll of salami from the walk in cooler to start preparation. Asked employee to wash hands even after washing hands inside the bathroom. Employee washed hands correctly.

Observed boiled egg (48F - Cold Holding); slice tomatoes (56F - Cold Holding); yellow cheese (53F - Cold Holding); cut lettuce (57F - Cold Holding) at the front counter prep cooler, as per operator placed on the line less than 4 hours. Operator placed items under ice until the technician comes out.
No paper towels or mechanical hand drying device provided at handwash sink. In the bathroom, operator replaced paper towels.
