Operator placed items under ice.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice machine chute with mold like substance in the lobby.
Carbon dioxide/helium tanks not adequately secured. Observed one of the CO2 tanks not secured by the mop sink.
