Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed the wiping cloth quaternary solution for the sanitizer near the hand sink on behind the front counter. At 0ppm. The operator replaced the sanitizer with a quaternary solution of 200ppm.

Employee used tobacco, ate, or drank then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed male employee taking a drink where prep table is across from three compartment sink and returning to work with out washing hands.
Handwash sink not accessible for employee use at all times. Observed hand washing sink right next to triple sink with a large black bowl in it. Operator removed black bowl to drying board.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed chocolate milk and ice water on prep table where slicer is. Observed female employee removing the drinks right away.
Employee eating while preparing food. Observed male employee chewing gum and preparing food. Had employee spit gum out and explained why.
Employee with no hair restraint while engaging in food preparation. Observed male employee with no hair restraint on.
