Items placed on time control.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Missing at mop sink.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. At front; coolers reading 52F and 54F. Coolers not keeping food at 41F or below.

Old labels stuck to food containers after cleaning. Observed one food container on storage rack with an old label stuck to side. Person in charge removed label and placed container in ware washing.
