Observed roast beef (48F - Cold Holding); chicken (55F/57F - Cold Holding); cold cuts (54F - Cold Holding); stuffing (46F - Cold Holding); tuna (46F - Cold Holding); ham (44F - Cold Holding); turkey (47F - Cold Holding); cheese (48F/47F - Cold Holding); beef (55F - Cold Holding); cut tomato (48F - Cold Holding); cut lettuce (45F - Cold Holding)in both front counter cold holding units. All products placed in unit at approximately 9:00. Temperatures taken at approximately 11:45. All products moved to the walk in cooler.
Duct tape used to repair nonfood-contact surface. Duct tape used on plastic bins holding sauces and food products in dry storage.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Cut tomato and cut lettuce on left cold bar both at 47F. Manager states removed both from walk in cooler approximately 30 minutes prior. Both products moved to walk in cooler to quickly chill.
Handwash sink used for purposes other than handwashing. Two pans and lid in handwash sink at triple sink. Items removed.

Per person in charge, all products placed in reach in cooler from walk in cooler at 8:30am. Temperatures taken by inspector at 10:25. Person in charge placed all products in walk in cooler.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Threaded mop sink faucet with attached hose missing vacuum breaker.
Food-contact surface soiled with food debris, mold-like substance or slime. Interior of ice chute on dining room soda machine has black slimy mold-like substance.
Cove molding at floor/wall juncture broken/missing. Cove molding missing along wall under chemical storage shelf. Two holes are present where floor meets wall.

Dead roaches on premises. One dead roach on right side of dining room reach in cooler. Person in charge removed and discarded dead roach.
Dumpster overflowing garbage. Both shared dumpsters are overflowing with garbage piled so lids cannot be closed.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of microwave on make line is soiled.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Counter under tea machine has debris. Walk in freezer gasket is soiled.
Open dumpster lid. Both lids on both shared dumpsters are open.
