Quaternary ammonium sanitizer used to sanitize clean in place equipment not at proper minimum strength. Do not use equipment not properly sanitized. Observed quaternary sanitizer in the bucket at front counter read 0 ppm, operator changed it and properly sanitized at 200 ppm.
No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. Wet mop not stored in a manner to allow the mop to dry. Observed mop not stored correctly to dry at the mop sink, operator moved.

Equipment and utensils not properly air-dried - wet nesting. Operator restocked to promoate air drying.
Exterior door has a gap at the threshold that opens to the outside. Rear door does not fully close from inside. Gap a bottom remains open unless employees walk around building to back and close from outside. This is a pest issue as well as a safety issue for employees.
In-use ice scoop stored on soiled surface between uses. Observed ice scoop in container that had debris and slime on bottom contacting scoop. Operator washed and sanitized both.
