Submit a license application and payment to DBPR/Bureau of Central Intake Unit, 2601 Blair Stone Rd., Tallahassee, FL 32399-0783. Establishment has applied for Licensing however needs to satisfy complaint observations before being licensable.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed chicken (50F - Cold Holding); cut tomatoes (55F - Cold Holding) left in shallow pans in reach in cooler with cover opened for less than two hours per person in charge. Person in charge closed cover too reach in cooler.
Dead roaches on premises. Observed three dead roaches on floor near back door. Person in charge swept up and removed dead roaches.
Hole in or other damage to wall. Observed walls by back door peeling and curling up from floor.
