Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee put on hat touching hair then proceeded to handle sandwiches after changing gloves without washing hands. Manager had employee wash hands and change gloves.
