Operator is already on warning for an extension till he end of March. Operator has stated owner has scheduled a class for mid-March for training.
Carbon dioxide/helium tanks not adequately secured. Observed three CO2 tanks not secure in the back of the kitchen.
Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tile is peeling near back of kitchen over chip boxes.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee with bracelet on while preparing sandwiches, operator had employee take off.
Equipment in poor repair. Observed ice chute that is part of the soda dispenser out in the dining room is not working correctly, operator stated work order has been put in and a sign out of service sign has been placed on it.
No conspicuously located ambient air temperature thermometer in holding unit. Observed no ambient air temperature thermometer in the front reach in cooler out in the dining area, operator put thermometer in cooler.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gasket around walk in cooler door is soiled.
