No soap provided at handwash sink. Observed no hand soap at the handwashing sink in the back prep area.
Ceiling/ceiling tile shows damage or is in disrepair. Observed a damaged ceiling tile above the restroom area.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup up on the shelves and the pipes in the walk in freezer.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed the interior of the microwave being soiled with food debris. Employee stated she will clean the interior of the microwave.
