No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Left a copy of Employee Health Agreement with employee to have Operator make copies and have all employees sign.

Personal backpack, cell phone and insulated tumbler were stored on a prep table adjacent to where an employee was preparing chicken. Personal items should be stored in a designated area, below and away from food and equipment.
