Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Inside warmer at front counter: meatballs (109F - Hot Holding). Manager stated the meatballs were heated this morning at 8am and placed in the warmer. The warmer is turned off. Manager turned on the warmer and reheated the meatballs to 202F during this inspection.
No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. No proof of food handler training available for any employees.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drink cup stored on top of microwave at kitchen entrance. Manager removed cup from the area during this inspection.
Open dumpster lid. Lids of both dumpsters at end of plaza left open.
Single-service items stored under sewer lines that are not shielded to intercept potential drips. Plastic sleeve containing single-use plastic cups stored below drain pipe below handwash sink at front counter. Manager removed cups from the area during this inspection.

Chicken is still partially frozen. Inspector had manager place the chicken into a cooler to continue to thaw.

Two rodent droppings on bottom shelf below fountain machine in customer dining area. 52 rodent droppings inside interior of cabinet behind front counter and on floor through hole inside bottom of cabinet, below handwashing sink. 21 rodent droppings on shelf below meat section of front counter, beside boxes containing single-use gloves.
No soap provided at handwash sink. Soap dispenser empty at handwashing sink near three-compartment sink.
Cardboard used to line nonfood-contact shelves. Cardboard used to line shelf below takeout container lids near back door of kitchen.
Cutting board has cut marks and is no longer cleanable. Cutting board on bottom shelf of clean dish rack has deep cut marks and is no longer smooth and easily cleanable.

Manager placed a sign on the back door instructing employees not to use the back door and a sign on the back of the restaurant instructing people to keep out of the area. Manager also placed crates on the ground behind the back door to keep people from stepping in the sewage.
Toxic substance/chemical improperly stored. Spray bottle of sanitizer of shelf beside takeout containers in cabinet behind front counter. Bottle of lotion on shelf above bottles of olive oil and vinegar in prep area of kitchen. Manager removed both chemicals from shelves during this inspection.
Food storage container/container lid cracked or broken. Cracked plastic pan lid on pan of chicken inside walk-in cooler.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Area around nozzles of fountain machine soiled with mold-like substance in customer seating area
