Exterior door has a gap at the threshold that opens to the outside. Observed light visible under back door. Also, self closer disconnected at back door.
Floor tiles missing and/or in disrepair. Floor tiles in back kitchen near mop sink in poor repair. Observed wear at front floor tiles on make line. Observed porous tiles in back kitchen storage area. Floor around grease trap and triple sink with debris buildup.

Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee making sandwiches answered soiled phone and immediately placed gloves on to prepare sandwiches without washing hands. Inspector coached employee on proper hand washing procedures. Employee washed hands correctly.
Employee rubbed hands together for less than 10-15 seconds while washing hands. Employee at front counter used hand washing sink to wash hands but only for approximately five (5) seconds. Inspector coached employee on proper hand washing procedures. Employee washed hands correctly.
