Food-contact surface soiled with food debris, mold-like substance or slime. Lobby area soda dispenser ice chute has mold like substance on the sides of the chute.
No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. No proof of food handler training for employees present.
Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Unable to dispense paper towels in women's restroom. Operator provided new roll.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Clean metal trays on shelf of prep area not inverted or covered. Operator covered top tray.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In the steam table at the front counter, meatballs (117F - Hot Holding) employee reheated over 165F.

Container of medicine improperly stored. Container of medicine on shelf above prep area. Person in charge moved to appropriate location.
Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee don gloves to begin working with food after rolling up coworkers sleeves without hand wash. Spoke to employee during inspection.
Rodent activity present as evidenced by rodent droppings found. Observed 5 rodent droppings in cabinet under self service soda dispenser in dining area. Person in charge began cleaning.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Meatballs at 102F. Person in charge began reheating.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Person in charge unable to provide written plan for biohazard cleanup.
Floor soiled/has accumulation of debris. Floors with debris buildup along walls, under shelving. Ceiling tiles in front area are porous.
Improper storage of maintenance equipment that interferes with cleaning. Observed generator, power washer, chainsaw stored in dry storage area, making it difficult to clean floors in area.
Toilet/urinal not flushing/functioning properly. Urinal out of order in men's restroom.
Water leaking from pipe and/or faucet/handle. Leak in walk in freezer near water lines with catch buckets below. Also, leak at soda dispenser collecting into trash can.
Wet mop not stored in a manner to allow the mop to dry. Two wet mops stored in mop sink.
