Accumulation of black/green mold-like substance in the interior of the ice machine/bin.

Handwash sink not in working order in food preparation/dishwashing area. Must be reinstalled in the same location where removed. Employee stated sink has been out of order for more than a year.
Manager or person in charge lacking proof of food manager certification. Manager not on duty but can not provide proof of certification states in General manager of properties office. Will send in email at a later date.

Carbon dioxide/helium tanks not adequately secured. Observed carbon dioxide/helium tank not secured in bar area.
No conspicuously located ambient air temperature thermometer in holding unit. Observed no ambient thermometer inside of walk-in cooler. Manager placed new ambient thermometer inside of walk-in cooler at time of inspection.
