Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee pick item up off of the floor with gloved hands, then packed food items in to go containers without hand wash. Person in charge and employee remove gloves and wash hands.
