Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Employee handled raw ground beef and changed gloves to then work with ready to eat lettuce without hand washing. Discussed with operator and observed proper hand washing with glove change.

During 8:00 AM line check food was observed by the operator to be at 38 F (ColdHolding). It can be determined that food was not above 41 F more than 4 hours. Operator moved food to walk in freezer and other functioning cold holding units to rapidly cool.
