Observed salmon just received and frozen in walk-in cooler, still in reduced oxygen packaging. Operator punctured all.

Employee failed to wash hands before putting on gloves to initiate a task working with food. 1. Employee performed work in office - then put on gloves and began working front line serving ready to eat food without washing hands. 2. Employee used cash register and gave customer change. Then put on gloves and proceeded to handle ready to eat food without washing hands. Reviewed proper hand washing and employees properly washed hands.
Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Triple Sink (Quaternary 0ppm). Operator drained sink and added new sanitizing solution. Quat 400ppm.
