Non-food grade paper/paper towel used as liner for food container. Clean wiping cloth used as liner for pan of raw scallops in reach in cooler and fried chicken in stand up warmer in back kitchen area. Manager removed cloths from products.
Stop Sale issued due to food not being in a wholesome, sound condition. In walk-in cooler, eleven (11) vacuum sealed packages of raw mahi fully thawed out in bag.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In flip top reach in cooler on cook line, shredded cheese (49F - Cold Holding); sliced cheese (48F - Cold Holding); cut melon (47F - Cold Holding); cut lettuce (48F - Cold Holding); coleslaw (49F - Cold Holding); ham (45F - Cold Holding). Manager stated all items were stocked about 3 hours prior and started putting all Time/Temperature control for safety foods in walk-in cooler on ice to bring down temperature. Ambient air of cooler at 51F. Also, near drink machines in lobby, butter (62F - Cold Holding). Manager stated it was put out 3 hours ago and was unaware it had to be held cold. Manager stated he will put it on time and throw it out in an hour.
Handwash sink not accessible for employee use at all times. In handwash sink in back kitchen area, spray bottle of cleaner and multiple steel scrubbers in sink. Manager removed from sink.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. See stop sale. In walk-in cooler, eleven (11) vacuum sealed packages of raw mahi fully thawed out in bag.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of middle oven in back kitchen soiled with food debris and soiled tin foil.

In stand up reach in freezer, plastic container of raw chicken stored above box of raw Philly meat. Operator moved raw chicken to bottom shelf.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In warmer unit in back kitchen area, sausage patty (132F - Hot Holding). Operator stated sausage was cooked 2 hours prior. Operator removed sausage from warmer and put in walk-in cooler to begin cooling process.
Spray bottle containing toxic substance not labeled. On chemical shelf, white spray bottle with no label. Operator wrote label on bottle.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Multiple water bottles/drink cans stored on shelves with food items throughout establishment. Operator removed all drinks and discarded.

Three employee cellphones stored on box of food cans next to boxes of salt. Operator moved cellphones. Also, multiple employee jackets stored hanging/on shelf with food products. Operator removed jackets and will find better spot to store employee belongings.

Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. At server station, Sanitizer Bucket (Quaternary 0ppm). Manager remade sanitizer bucket and rechecked at 150ppm.

Operator stated items haven't been in cooler for very long. Operator will keep door closed on Reach in cooler to chill foods.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Cooked chicken (126F - Hot Holding); house made gravy (126F - Hot Holding) in food warmer in back kitchen area. Operator raised temp of food warmer to reheat back to 165F
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Two threaded hose bibbs on exterior wall near restrooms missing vacuum breaks.
