Observed large mass of ice on fan area. Operator removed both TCS foods and placed in walk in cooler to chill.
Toxic substance/chemical improperly stored. Spray bottle containing food grade grill release stored on chemical shelf with spray bottles containing toxic substance near dry food storage. Operator removed food grade spray away from chemical spray bottles to properly store.
Food-contact surface soiled with food debris, mold-like substance or slime. Cutting board on cookline at make table soiled/ stained.
Handwash sink used for purposes other than handwashing. Hand wash sink next to managers office used to dump cup of ice water. Operator melted ice from hand wash sink.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Reach in cooler below make table on cookline unable to maintain temperature of 41F and below. Ambient temperature 48F. Observed,large mass of ice over entire fan area. Operator removed TCS foods from reach in cooler and stated she will have it serviced.
