Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee dipped gloved hands in sanitizer bucket and then grabbed tortillas and placed on warmer. Had another employee translate to her to remove gloves, throw away tortillas and wash hands prior to putting on new gloves.
