Observed no proof of employees health reporting health reporting agreements. E-mailed copy of to manager who printed copies and had all employees read and sign the health agreements.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling vents in back food storage area with excess soil and dust.
Floor not cleaned when the least amount of food is exposed. Observed floor beneath and next to fryer with excess soil, grease and food debris.

Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the underside of soda dispenser with black mold like substance. Operator was in the process of cleaning the area.

Handwash sink not accessible for employee use at all times. Observed a trash can obstructing access to the handwashing sink behind the front counter. The operator moved the trash can.
In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed the scoop handle used for cheese not stored above the food. The operator properly stored the scoop handle.
Marked exit/path to marked exit blocked. For reporting purposes only. Observed the rear exit blocked by discarded cardboard boxes. The operator removed the cardboard from the exit path.

Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. Observed a screwdriver stored on top of pepper packets near the drive-thru. The operator properly store the screwdriver.

Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed the ceiling vent in front of the walk in cooler soiled with dust.
In-use utensil not stored on a clean portion of food preparation or cooking equipment. Observed in-use scoop for uncooked pasta stored on top of a soiled container lid. The operator removed the scoop for cleaning.
Open dumpster lid. Observed the dumpster lid is blocked by the wall and unable to close.
