Manager unable to provide proof three employees that have just started within two weeks have been informed of their health reporting responsibilities. Inspector sent manager DBPR health reporting form and she will have new employees fill it out.
Equipment and utensils not properly air-dried - wet nesting. Observed multiple stacks of metal pans stacked before properly air dried.
Floor, wall and/or ceiling soiled/has accumulation of debris. Floor under cook line equipment soiled with grease and food debris. Also, floor under reach in cooler soiled with food debris. Also, floor under drink station in drive thru soiled with residue build up. Also, floor/wall under three compartment sink soiled with residue build up. Also, ceiling tiles above handwash sink/three compartment sink soiled with dust build up.
Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup up on door and door frame of walk-in freezer.

Certified Food Manager or person in charge failed to verify employee health, exclusions or restrictions. Manager stated that three new employees started last week, manager not able to provide documentation that employee responsibilities were explained to them, employees did not have food handler certifications. Sent employee health agreements to Manager who printed them out and had employees sign them.
Food-contact surface soiled with food debris, mold-like substance or slime. Nozzles on tea dispensers near drive through window soiled with calcium like substance. Old coffee residue build up on coffee maker Nozzle near drive through window.
No soap provided at handwash sink. No handwashing soap/ dispenser in men's restroom.
Spray bottle containing toxic substance not labeled. Observed unlabeled pump spray bottle with purple chemical in it, manager was not aware of what chemical was in it.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Multiple ceiling tiles in kitchen area soiled with dust.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee drink on food prep table next to soda dispenser behind front counter. Manager relocated drink.
