Manager or person in charge lacking proof of food manager certification. Owner stated she has misplaced her certificate and cannot locate it.
No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Printed a copy out for owner and had it signed by end of inspection.
No waste receptacle installed at handwash sink provided with disposable towels. Observed no waste receptacle owner brought one over.

Only use single-service items to serve food to customers until approved sanitizer is available for warewashing. Observed operator using splashless bleach as sanitizer. Operator unit operating at this time and will secure the correct chlorine sanitizer before reopening.
