Mobile food dispensing vehicle obtaining water from an unapproved source. MFDV obtains water from building on site St.Johns Marine Inc, not approved commissary location.
Handwash sink not accessible for employee use at all times. Hand wash sink blocked by food prep over sink. Employee removed the food to allow access.
Mobile food dispensing vehicle disposing liquid waste improperly. MFDV disposes of waste water via buckets to tub inside of storage building St.Johns Marine inc.
Nonself-sufficient mobile food dispensing vehicle not reporting to commissary as required. MFDV is utilizing empty building behind regular set up location to acquire potable water and dump waste water also to store frozen time/temperature control food for safety as well as single service containers and non time/temperature control food for safety.
Food stored outside of establishment. In storage building behind MFDV set up location, packs of bread stored in storage room as well as chest freezer in storage room.

Raw animal food stored over/not properly separated from ready-to-eat food. Raw chorizo stored over bucket of red sauce in standing reach in cooler, chorizo moved.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. shredded mozzarella cheese (46F - Cold Holding) in bottom of flip top reach in cooler, per employee bag has been taken out of cooler several times over the last 30 minutes.
Dead roaches on premises. 2 dead roaches around water heater. 3 dead roaches inside cardboard box over service window. All dead roaches cleaned up and discarded.
Employee with no hair restraint while engaging in food preparation. 1 employee making food on grill.
Outer openings not protected during operation and vermin and/or environmental cross contamination present. 2 flying insects inside food truck with door propped open.
Single-service articles improperly stored. Box of to go cups on floor in building next to food truck.

In-use utensil stored in standing water less than 135 degrees Fahrenheit. Utensils on cook line in water at 127F.
Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. Chlorine sanitizer bucket at 0ppm stored on the floor. Operator added bleach and relocated the sanitizer bucket.
