Commercially processed reduced oxygen packaged tuna bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Per operator, fish stored in packaging for approximately two hours. Operator had employee remove fish from packaging to allow oxygen flow.
Covered waste receptacle not provided in women's bathroom. Observed no trash can with lid inside unisex restroom.
No handwashing sign provided at a hand sink used by food employees. Observed no handwashing sign inside restroom.
