Nonexempt fish offered raw or undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded. Fish and shrimp served raw in ceviche, operator unable to provide proof of parasite destruction.
Raw animal food stored over/not properly separated from ready-to-eat food. Two flats with raw shell eggs on shelf over cups of sour cream. Moved during this inspection.
Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Pan of rice 104F and beans 110F on top of rotisserie machine and shelf over prep table, cooked one hour prior. Employee moved to reach in cooler.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Missing for one new employee currently working. Employee signed form during this inspection.
