Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishwasher (Chlorine 0ppm). Observed hose not fully in bucket of sanitizer. Operator readjusted tube and reran and retested at 100ppm.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. house made guacamole w/ cut tomatoes, house made pico (45F-46F - Cold Holding)in top of flip top reach-in cooler ne t to food warming table. Operator stated it may have been left open during lunch. Operator removed both foods and placed in bottom of reach-in cooler to chill.

Also, an employee used same hand wash sink to fill container with water. Operator discussed with employee sink is used only for handwashing.
No paper towels or mechanical hand drying device provided at handwash sink. No paper towels in unisex restroom across from storage cabinets. Operator replenished paper towels.
No proof of required state approved employee training provided for any employees. Operator unable to provide proof of employee food safety training. He stated he had it at his other location.

Operator unable to provide proof that employees are informed of reporting symptoms related to food borne illnesses to person in charge. Distributed health form to operator during this inspection and operator had employees on duty read, sign and date employee health form.

Operator stated the lid was opened during lunch. Operator stated he will keep lid closed to chill pico. Also, cooked chicken (44-45F - Cold Holding) in walk-in-cooler. Operator stated he pulled the pans of chicken out during lunch rush. Operator stated he will keep the pans of chicken in walk-in-cooler to chill.
