Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed multiple food items held above 41F in walk in cooler at 4:15: chicken thighs 49F, chicken wings 59F, liver 51F, chicken legs 53F, catfish 68F, chicken bites 51F. Per manager walk in cooler door has been opened frequently from 11:30 to 3:30. Walk in cooler ambient temperature displays: 56F. See stop sale.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed mold like substance inside ice machine bin.
Single-service articles improperly stored. Observed to go cups stored on floor in back of restaurant. Manager put on shelf.
