Food-contact surface soiled with food debris, mold-like substance or slime. Portable cutting board on the prep line has food residue staining around the cutting grooves. Deflector plate of the bulk ice bin has mold like substance spots. Operator bleached and scrubbed the cutting board.

Carbon dioxide/helium tanks not adequately secured. By the bag in box sodas, one carbon dioxide tank stored not secured.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Blue ice bucket at the front counter not stored inverted or covered for protection.
Employee personal food not properly identified and segregated from food to be served to the public. Employee personal food stored in reach in cooler over sliced cheese. Operator relocated the personal food.
Equipment in poor repair. Standup reach in cooler by the bag in box soda has a torn cooler door gasket.
Floors, walls and ceiling soiled/has accumulation of debris. Build up of grease on floor under main cook line equipment. Build up of dust on ceiling by attic door.
Food stored on floor. Fryer oil stored on the floor by the rear exterior door.
In-use utensil not stored on a clean portion of food preparation or cooking equipment. Tongs stored in divider between grill and fryers with grease build up. Operation relocated the tongs.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Filters on hood system over the fryers have build up of dust. Standup reach in freezer door gaskets has mold like substance build up. Divider between fryers and grill has build up of grease debris.
Old labels stuck to food containers after cleaning. Old sticker on clean container on shelf by the three compartment sink. Operator removed the sticker.
Reach-in cooler interior/shelves have accumulation of soil residues. Interior base of the cook line reach in cooler has build up of residue/debris. Per operator reach in cooler not in use.
Single-service articles improperly stored. In the office area dry storage room, single service carry out containers in box stored on the ground.
