Per operator, not prepared or portioned today. Per operator, out of temperature for approximately 1.5 hours. Operator returned to cooler.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. cooked potatoes (128F - Hot Holding) Observed in hot holding unit on cook line. Per operator, out of temperature for approximately 30 minutes. Operator turned up temperature on hot holding unit.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed splitter and hose attached at mop sink with no vacuum breaker. Operator removed hose.
Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. prime rib (62F - Cold Holding) at 1:45 since 12:45 - 62F at 2:05. At current rate of cooling product will not reach 41F within 4 hours. Operator put in reach in freezer to quick chill.
No paper towels or mechanical hand drying device provided at handwash sink. Observed Paper towel dispenser inoperable due to being out of batteries. Operator provided paper towels.
