Nonself-sufficient mobile food dispensing vehicle operator failed to submit a Commissary Notification form to the division for each commissary prior to use. OnBase shows a commissary letter from a different address. Than from what new food trucks location. Manager emailed new commissary letter to inspector at time of inspection.
Operator is not properly tracking/marking the number of days ready-to-eat, time/temperature control for safety food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Steak cooked 5-21-24 label Cooked chorizo 5-15-24 label cook beef 5-14-24 cook pork 5-22-24 cook beef 5-15-24 reach in white cooler. No 2nd label once pulled from freezer.
