Nonself-sufficient mobile food dispensing vehicle operator failed to submit a Commissary Notification form to the division for each commissary prior to use. Operator stated he is now cooking at a location in Ocala and bringing food to truck.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: in the loboy reach-in cooler(chef table) Pork 47°, beef 47°, chicken 46°. ALSO, in the flip top shred cheese 47°. The Mfdv is Ambient air temperature 89°.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Observed lack of a plan.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed: the can opener has dried food debris on it.
Handwash sink used for purposes other than handwashing. Observed the only hand wash sink was full of plastic dishes. The sink was emptied. Staff started washing hands.

No Heimlich maneuver/choking sign posted. Observed: lack of a choking poster.
