Wiping cloth chlorine sanitizing solution not at proper minimum strength. Observed reading at 10pm, owner changed out solution and new reading 100ppm.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed on the flattop in a container: Beef (105F - Hot Holding). Operator stated it had been out for approximately 30minutes. Operator placed the top half of the beef on the grill to heat up.
Employee with no hair restraint while engaging in food preparation. Observed an employee cook food with no hair restraint.

Stored on bottom touching floor of cooler which is not cooled to optimal temperature. Must place shelf on bottom so air flow can circulate fully around the foods. Rice must cook from Q135F tom41F within 6 hours. New coolers not set at cool enough temperature to optimize cooling. Operator will turn thermostat down to 34F and place shelved over bottom, not put food containers in direct contact with floor of cooler. Ambient air of cooler at 39F. Floor of cooler at 47 to 50 F.

Stored on bottom touching floor of cooler which is not cooled to optimal temperature. Must place shelf on bottom so air flow can circulate fully around the foods. Ambient air thermometer in holding unit not accurate within plus or minus 3 degrees Fahrenheit. Observed thermometer reading 32F mall foods in cooler 39F to 41F. Replace thermometer.

No paper towels or mechanical hand drying device provided at handwash sink. At front counter and near ice machine operator added paper towels.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed operator has forms but none have been signed and dated by employees. Reexplained the form to the operator.
No handwashing sign provided at a hand sink used by food employees. At handsink at front counter used for ice machine. Operator added sign at inspection.
