Raw animal food stored over/not properly separated from ready-to-eat food. In stand up reach-in cooler, raw hamburger stored over hot dog buns. Operator rearranged to correct storage levels.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit.in bottom of flip top reach-in cooler at cook line, wings (49F - Cold Holding) Operator stated he had wings out for cooking. Moved wings to large reach-in cooler.

No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at kitchen handwash sink. Operator supplied. In restroom, no paper towels, soap or sign. Operator supplied soap and towels.
Spray bottle containing toxic substance not labeled. On shelf above water heater, spray bottle identified as bleach water not labeled. Operator labeled.
Carbon dioxide/helium tanks not adequately secured. At front counter soda dispenser, one tank not secured.

Observed: Establishment operating without a license from the Division of Hotels and Restaurants. Operator must obtain a public food service license from the Division of Hotels and Restaurants within 60 days. Submit a license application and payment to DBPR/Bureau of Central Intake Unit, 2601 Blair Stone Rd., Tallahassee, FL 32399-0783. Observed food being served to customers. Operator stated he is applying for a license today. Priority: High Priority 9/5/2023- Called office, no application submitted 10/12/2023- Application approved, not able to license due to mop sink removed.
Observed: No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days.
