Raw animal food stored over/not properly separated from ready-to-eat food: four door reach-in cooler on cook line had a pan of raw shrimp over a pan of cooked shrimp. Cook moved the cooked shrimp to another shelf. Additionally, three flats of raw shelled eggs were in the same reach-in cooler over coleslaw and shredded lettuce. Cook took the eggs to the walk-in cooler.
Spray bottle containing toxic substance not labeled: spray bottle located in the wait station area has chlorine sainting water in it but no label. Assistant General Manager immediately put a label on it.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Line low boy cooler - Raw shrimp 44F ( less than 4 hrs. ) Advised operator to ice for rapid cooling. Cold holding industry bulletin provided to operator Front porch.
Unsealed concrete floor in food preparation, food storage, warewashing area. Unsealed wooden floor in exterior walk in cooler.

Handwash sink not accessible for employee use at all times. Handwashing sink in ware washing area filled with dishes and equipment. Handwashing also has dishmachine chemicals in front of it, blocking access. Operator removed items from sink during inspection.
No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. A list of accredited food manager certification examination providers can be found at http://www.myfloridalicense.com/DBPR/hotels-restaurants/food-lodging/food-manager/ 5 employees present, no food manager present. Per operator, food manager will be arriving later in the day.
Equipment in poor repair. Walk-in cooler door in disrepair: door does not fully seal when closed, but is still keeping food cold.
Unsealed wooden floor in food preparation, food storage, warewashing area or bathroom.
A return visit. The problems from the previous inspection had been dealt with.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Some debris buildup on right side of ice sheild.
Carbon dioxide/helium tanks not adequately secured. Tanks along wall near soda fountain syrup rack not secured.
Ceiling/ceiling tile shows damage or is in disrepair. Ceiling tiles throughout kitchen are damaged/soiled.
