Vacuum breaker was incorrectly installed on the chemical side. Operator removed vacuum breaker and properly installed on non chemical side of splitter during this inspection.
In-use knife/knives stored in cracks between pieces of equipment. Observed two knives stored in between to prep tables in meat cutting area. Employee removed both knives and placed in warewashing area to be washed.

Raw animal food stored over/not properly separated from ready-to-eat food. In walk in cooler on speed rack, raw-ready to eat smoked salmon stored over cooked vegetables. In walk in cooler on shelves, raw beef stored over cooked beef. Items moved and stored correctly.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On cook line in kitchen, two (2) packages of butter 57F. Butter placed on counter approximately 15 minutes prior. Butter moved to walk in cooler to cool.
Toxic substance/chemical improperly stored. On dry storage shelf in kitchen, butane cans stored next to lemon juice. Butane moved and stored correctly.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. No written plan available. Inspector emailed Form 5030-104 (Clean Up Instructions for Vomitting/Diarrhea).
Handwash sink inoperable. Hand washing sink in women's restroom (left side) is not working. Per management, this restroom can be used by employees.

Employee placed gloves on to finish task.
