Vacuum breaker missing/incorrectly installed at mop sink faucet or on fitting/splitter added to mop sink faucet. On mop sink with added double splitters, vacuum breaker missing from non-chemical side with attached red hose. Operator has repair person called to complete installation.
Buildup of food debris/soil residue on equipment door handles. Black and pink substance observed on lid handle and rim along the handle on both doors of the ice machine. Employee cleaned both lids during inspection.
Floor tiles missing and/or in disrepair. Multiple floor tiles missing and/or chipped in dishroom and at back exit door. Operator states repair materials are on site and will be repaired next week.
Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup up observed in the walk-in freezer.

Water draining onto floor surface. Water draining on the floor under the dishroom table. Operator states the faucet broke and will be repaired today.

Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed cook in kitchen operating a cell phone. Cook then put phone in his pocket and placed on gloves and began to place food in the fryer. Cook did not wash hands before placing on gloves.
Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed raw beef (48F/47F/47F/44F - Cold Holding) in far right top drawer at steak cooking area. Per employee, all beef placed in drawer at 11:00pm. Inspector took temperatures at 3:24pm. Also, raw chicken (61F/62F/61F/63F - Cold Holding) on speed rack at prep area. Per employee, chicken removed from the cooler at 8:30am. Inspector took temperature at 3:40pm.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. See stop sale. Observed raw beef (48F/47F/47F/44F - Cold Holding) in far right top drawer at steak cooking area. Per employee, all beef placed in drawer at 11:00pm. Inspector took temperatures at 3:24pm. Ecolab Sink and Surface Sanitizer used in three compartment sink tested at 170ppm. Manager adjusted equipment and retested at 700ppm. Manager made service call during inspection to ensure continually operation of chemical system. Also, Sanitizer wiping cloth solution bucket stored on the floor on expo line and in kitchen across from the stove. Manager moved solutions to shelving.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In bottom refrigerated drawer under the char grill, pork chops at 46F and beef at 46F. Both products brought from walk in cooler and placed in drawer approximately 2.5 hours prior. Both products moved to walk in cooler and both products at 43F and 42F respectively at end of inspection.
Handwash sink used for purposes other than handwashing. Wiping towel in hand wash sink in dish room. Employee removed towel. Another employee rinsed another wiping cloth out in dishroom handwash sink.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Vents above three compartment sink are dusty. Fan guards in meat walk in cooler have debris.
Food storage container/container lid cracked or broken. Multiple plastic food storage containers on shelf at back prep sink are cracked and some have pieces of the side of the container missing/broken off.

Ceiling/ceiling tiles/vents/walls/floors soiled with accumulated food debris, grease, dust, or mold-like substance. In women's restroom, ceiling tiles surrounding the vent are dusty. Two stained ceiling tiles at back of room. Men's restroom vent is dusty. Vents above storage rack in dishroom and above three compartment sink are dusty.
Equipment and utensils not properly air-dried - wet nesting. Wet nesting observed on storage rack in dishroom. Water present in between stacked metal inserts.
Water leaking from pipe and/or faucet/handle. Water leaking at bottom of pre rinse sink in dishroom.
