Food-contact surface soiled with food debris, mold-like substance or slime. At prep area, green cutting board has black stains. Manager removed from use. At wait station, multiple flavor nozzle on right side dispenser has build up. Manager cleaned during inspection. At prep area, food dicer has food debris. Manager had employee clean.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. In kitchen, interior of ice machine has black stains on top interior plate. At wait station on right side soda ice bin, top plate has black mold-like substance.
Employee drinking from an unenclosed beverage container in a food preparation/clean dishware area and the container does not have a dispensing mechanism to prevent hand contact with lip contact area of drink container. At prep area, employee drink has no lid or straw. Manager discarded.
