Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee on cook line remove gloves and start to put on new gloves with no hand washing in between. I spoke with employee on proper handwashing techniques between gloves changes. He washed hands and put on new gloves. I watched him do correct hand washing technique 2 other times.

Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within 2 hours. Walk in cooler: cooked rice made at 9:35, time taken 11:45am (85F - Cooling). Manager put in freezer and re-temped at 40 at 1pm
Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee wash their hands with gloves on and handled clean dishes. Manger coached employee and they changed their gloves and washed hands
