Establishment conducts non-continuous cooking of raw animal foods without monitoring and documenting initial heating time and temperature, cooling times and temperatures, cold holding temperature and final cooking temperature. Non-continuous cooking of chicken observed in flip top reach-in cooler. Written procedures available however, no documentation available for initial heat/temperature, cooling time/temperatures and final cooking temperature. Operator decided to discontinue non-continuous procedures.
Spray bottle containing toxic substance not labeled. Unlabeled spray bottle with clear liquid by kitchen microwave. Operator identified contents as water and labeled bottle.
Equipment in poor repair. Gasket is torn on chest freezer by back exit.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishmachine tested at 10ppm chlorine. Operator adjusted machine and retested at 50ppm chlorine.
Establishment conducts non-continuous cooking of raw animal foods without written procedures approved by the division. Establishment cooks raw chicken for approximately five minutes then cools and fully cooks the chicken when ordered. No written procedures available. Provided DBPR Form HR 5022-102 and 5022-101 and assisted operator with completion.
Floor soiled/has accumulation of debris. Flooring throughout kitchen and ware washing and under all equipment is soiled.

Operator states garlic placed out at 10:30. Temperature taken at 12:00. Operator placed garlic in cooler and temperature approximately 20 minutes later at 45F. Observed pork (48F - Cold Holding); raw chicken (46F - Cold Holding); rice (50F - Cold Holding) in stand up reach in cooler. Per operator, all products in unit from Saturday.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Sanitizer solution at kitchen handwash sink tested at 200+ppm chlorine. Operator remade to 100ppm chlorine.
