Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed 1 piece of fish fillet almost defrost inside the sealed bag. Employee opened the sealed bag immediately.
Floor tiles missing and/or in disrepair. Observed cracked tiles on kitchen floor.
