Employee dried hands on clothes/apron/soiled towel after washing. Observed employee wash his hands and then dry them on a soiled towel. Operator had employee wash his hands again and dry them with paper towels.
Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee cracked raw eggs and then changed his gloves without washing his hands and then proceeded to handle clean utensils. Operator had employee wash his hands and change his gloves.
Non-food grade paper/paper towel used as liner for food container. Observed paper towels used to line containers of spices and food in the prep area and front counter area. Operator removed paper towels from containers. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.

Observed raw calamari stored over cooked vegetables and ready to eat sauces in the prep make table on the cook line. Operator removed the vegetables and sauce.

Observed egg noodles stored at room temperature to the right of the cook line as: Rice noodles (67F - Cold Holding). Operator stated they had been out for approximately 1-1/2 hrs. Operator placed egg noodles in the reach in cooler. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.
Wiping cloth solution exceeding the maximum concentration allowed stored in a location that could result in the cross contamination of food, equipment, utensils, linens, single-service, or single-use articles. Observed sanitizer solution on the cook line: Sanitizer Bucket (Chlorine +200ppm) Operator discarded solution and remade bucket, new reading at 100ppm chlorine.
Handwash sink used for purposes other than handwashing. Observed employee using hand wash sink to rinse dirty utensils. Observed no left over debris in hand wash sink.
Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Observed approximately 15 dead roaches in a glue trap on a preparation table next to cook line. Operator discarded the trap.
