Food-contact surface soiled with food debris, mold-like substance or slime. Food contact service cooking pot on stove in kitchen with food and grease build up in and on outside pot. Fryer backsplash has grease and food debris build up and needs to be cleaned.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed dishwasher machine with 0ppm. Per Operator: using three compartment sink to wash, rinse, and sanitize and will not use dishwasher machine until repaired and sanitizer is at 50ppm.
