Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed at Main prep table raw tuna 47F, tofu 46F, cooked shrimp 52F. Per operator food not prepared or portioned today. Per operator Food held in unit less than four hours. Operator placed foods back into cooler to quick chill.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed cooked rice 126F. Per operator food held in unit less than four hours. Observed rice warmer not plugged in. Operator plugged rice warmer in.
Manager or person in charge lacking proof of food manager certification.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. No Vacuum breaker observed, splitter attached at mop sink.
No paper towels or mechanical hand drying device provided at handwash sink. At hand wash sink in prep kitchen.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Dishwasher (Chlorine 0ppm) Advised operator to sanitize via triple sink quaternary sanitizer 200-400ppm.
Single-service articles improperly stored. To go trays stored under hand wash sink in customer service counter area.
