Food-contact surface soiled with food debris, mold-like substance or slime. Observed food debris on the slicer in the guard and on the blade.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed duct tape on the inside of the machine on the ice cube drop tray. Duct tape is not easily cleanable and the wall had a mold like substance on it.
Carbon dioxide/helium tanks not adequately secured. Observed CO2 tanks behind bar not secured.
Ceiling tile missing. Observed missing ceiling tile in back storeroom past the triple sink.

Hood ventilation system inadequate as evidenced by grease accumulation on walls/ceiling. Observed heavy grease accumulation on the walls and ceiling of the hoods over the deep fryers.
Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed metro shelves in walk in cooler rusting.
